Астане (Республика Казахстан).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ данных о лекарственной чувствительности МБТ к противотуберкулезным препаратам первого ряда по классификации Всемирной организации здравоохранения, полученных при исследовании мокроты 449 пациентов c бактериовыделением, поступивших на стационарное лечение в противотуберкулезный диспансер г. Астаны в 2017 г. Лекарственную чувствительность МБТ идентифицировали фенотипическим (Bactec) и генотипическим (Hain-test) методами.</p></sec><sec><title>Результаты</title><p>Результаты. Исследование показало, что как среди впервые выявленных пациентов с туберкулезом, так и среди пациентов с рецидивом доминировала резистентность к изониазиду (H), составившая 87,4% (76 из 87) и 91,1% (41 из 45) случаев соответственно. Резистентность к H регистрировали чаще, чем к стрептомицину. H-устойчивые штаммы МБТобнаружены достоверно чаще, чем рифампицин-устойчивые: 76/226 (33,6%) против 18/226 (8,0%) среди всех впервые выявленных пациентов (p &lt; 0,01) и 41/133 (26,1%) против 5/133 (3,2%) среди всех рецидивов (p &lt; 0,01).</p><p>Среди впервые выявленных пациентов с туберкулезом с лекарственной устойчивостью возбудителя частота выявления рифампицин-устойчивых штаммов МБТ была сопоставима с частотой этамбутол-устойчивых. Идентифицирована низкая частота выявления устойчивости к пиразинамиду.</p><p>Проведенный анализ подтвердил, что при назначении лечения больным туберкулезом необходимо детальное изучение спектра резистентности к противотуберкулезным препаратам первого ряда.</p></sec></abstract><trans-abstract xml:lang="en"><p>The objective of the study: to study the prevalence of resistance to first line drugs among tuberculosis patients in Astana (Kazakhstan Republic).</p><sec><title>Subjects and methods</title><p>Subjects and methods. Data on resistance to first line drugs according to the WHO classification were analyzed, they included sputum test results of 449 patients with bacillary excretion who received in-patient treatment in TB Dispensary in Astana in 2017. Bactec and Hain test were used for drug susceptibility testing.</p></sec><sec><title>Results</title><p>Results. The study showed that among new tuberculosis patients and relapses, resistance to isoniazid (H) dominated and made 87.4% (76 out of 87) and 91.1% (41 out of 45) of cases respectively. Resistance to H was registered more often versus resistance to streptomycin. H-resistant strains were detected confidently more often than rifampicin resistance: 76/226 (33.6%) versus 18/226 (8.0%) among all new cases (p &lt; 0.01) and 41/133 (26.1%) versus 5/133 (3.2%) of all relapses (p &lt; 0.01).</p><p>Among new cases with drug resistant tuberculosis, the frequency of rifampicin resistant strains was compatible with the frequency of resistance to ethambutol.
