В исследование включено 62 пациента с ХОБЛ и СОАГС, диагностированным в соответствии с рекомендациями GOLD (2017). Клинико-функциональное обследование включало определение показателей функции внешнего дыхания и 6-минутного шагового теста, валидизированного опросника шкалы одышки Medical Research Council (MRC) с последующим расчетом индекса массы тела и индекса BODE.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. У пациентов с ХОБЛ при наличии СОАГС отмечаются утяжеление степени интенсивности одышки, углубление функциональных обструктивных нарушений, снижение толерантности к физической нагрузке. Течение заболевания сопровождалось большим числом обострений, требующих госпитализаций, у 77,4% больных ХОБЛ с СОАГС были заболевания сердечно-сосудистой системы и у 95,2% ‒ ожирение. Оценка интегрального индекса BODE у пациентов с ХОБЛ и СОАГС показала, что лишь 35,5% из них имеют процент 4-летней выживаемости более 57%. Установлено наличие обратной корреляционной связи сильной силы между индексом BODE и показателем максимальной десатурации (r = -0,78) и обратной корреляционной связи средней силы с SaO2 (r = -0,37). В результате 12-летнего наблюдения (2008-2019 гг.) за больными ХОБЛ с СОАГС установлена летальность в 22,2% случаев, преимущественно среди больных ХОБЛ с тяжелой степенью СОАГС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The objective</title><p>The objective: to evaluate the prognosis of 4-year survival in patients with chronic obstructive pulmonary disease (COPD) and concomitant obstructive sleep apnea-hypopnea syndrome (OSAHS).</p></sec><sec><title>Subjects and Methods</title><p>Subjects and Methods. 62 patients with COPD and OSAHS were enrolled in the study, they all had been diagnosed in accordance with the recommendations of GOLD (2017). The clinical and functional examination included assessment of respiratory function parameters and a 6-minute step test, a survey using the validated questionnaire of the dyspnea scale by Medical Research Council (MRC) followed by the calculation of body mass index and the BODE index.</p></sec><sec><title>Results</title><p>Results. Patients with COPD and concomitant OSAHS demonstrated higher intensity of dyspnea, deterioration of functional obstructive disorders, and lower exercise tolerance. The course of the disease was characterized by multiple exacerbations requiring hospitalization, 77.4% of COPD patients with OSAHS suffered from cardiovascular diseases and 95.2% were obese. Evaluation of the integral BODE index in COPD patients with OSAHS showed that only 35.5% of them had a 4-year survival rate exceeding 57%. A strong inverse correlation was established between the BODE index and the maximum desaturation rate (r = -0.78) and the moderate inverse correlation was established between the BODE index and SaO2 (r = -0.37).
