В ретроспективное исследование включено 108 пациентов 18 лет и старше, получавших Tpp в интенсивную фазу химиотерапии (ХТ) туберкулеза (ТБ) легких с МЛУ, пре-ШЛУ, ШЛУ. Вторую группу наблюдения составили 63 пациента, начавших интенсивную фазу противотуберкулезной терапии без включения тиоуреидоиминометилпиридиния перхлората. Оценка эффективности лечения проводилась через 2, 4 и 6 месяцев после начала химиотерапии по трем критериям: купирование клинических проявлений, положительная рентгенологическая динамика, прекращение бактериовыделения. Для оценки коморбидности рассчитывался индекс коморбидности Charlson.</p></sec><sec><title>Результаты</title><p>Результаты. Разделение пациентов на подгруппы по величине индекса коморбидности продемонстрировало зависимость от него эффективности лечения ТБ по общепринятым критериям. Пациенты с более высоким коморбидным индексом позже достигали улучшения при лечении ТБ. Сохранение бактериовыделения было характерно для пациентов при полиморбидной патологии и наркотической и алкогольной зависимости, а также при сахарном диабете.</p></sec><sec><title>Заключение</title><p>Заключение. Оценка эффективности противотуберкулезных препаратов у коморбидных пациентов является сложной задачей, учитывая это, необходимо прогнозировать их взаимовлияние. В нашем исследовании изучались особенности применения тиоуреидоиминометилпиридиния перхлората для лечения ТБ у коморбидных пациентов. Этот препарат уже успешно используется в условиях реальной практики. Полученные в этом исследовании данные также свидетельствуют о высокой эффективности режимов химиотерапии с его включением и у коморбидных пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The objective</title><p>The objective: to evaluate the efficacy of incorporating thioureidoiminomethylpyridinium perchlorate (Tpp) into the chemotherapy regimen for multidrug-resistant, pre-extensively drug-resistant and extensively drug-resistant (MDR, pre-XDR, and XDR) tuberculosis (TB) in patients with comorbidities.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. This retrospective study included 108 patients aged 18 years and older who received Tpp during the intensive phase of chemotherapy for pulmonary MDR-TB, pre-XDR, and XDR-TB. The second group consisted of 63 patients who started the intensive phase of anti-tuberculosis therapy without the inclusion of thioureidoiminomethylpyridinium perchlorate. Treatment efficacy was evaluated at 2, 4, and 6 months after initiation of chemotherapy based on three criteria: resolution of clinical symptoms, positive radiological dynamics, and cessation of bacterial excretion. Comorbidity was assessed using the Charlson Comorbidity Index.</p></sec><sec><title>Results</title><p>Results. Subgroup analysis based on the comorbidity index demonstrated that TB treatment efficacy assessed by standard criteria depended on the index value. Patients with a higher comorbidity index score achieved clinical improvement at a later stage. Persistent bacterial excretion was typical for patients with multimorbidity, substance and alcohol use, as well as diabetes mellitus.</p></sec><sec><title>Conclusion</title><p>Conclusion. Evaluating the efficacy of TB drugs in patients with comorbidities remains a challenging task; therefore, it is essential to predict potential drug-to-drug interaction. This study investigated the specific aspects of using thioureidoiminomethylpyridinium perchlorate for TB treatment in this complex patient population.
